A dental bridge is a tooth replacement option that fills gaps by anchoring a replacement tooth (pontic) to adjacent teeth using retainers. These retainers can be crowns or wings.

If crowns are used as retainers, the supporting teeth are prepared (shaped) just like they are for individual crowns. This creates the space for the bridge.

 

If wings are used, the supporting teeth may need minimal or no adjustments at all. The bridge is custom made in a dental lab based on the measurements of the supporting teeth. It is then cemented in.

Types of Dental Bridges

Conventional or Traditional Bridges

These bridges use crowns on both sides of the gap to support the pontic, for maximum stability. They can be made of precious metal, porcelain-fused-to-metal or all-ceramic materials like lithium disilicate or zirconia.

Cantilever Bridges

Cantilever bridges are supported by a retainer on one side of the gap (usually the backmost tooth). Less common than traditional bridges due to reduced stability which can compromise the supporting tooth.

Maryland Bridges or Resin-Bonded Bridges

Maryland bridges use a wing-like retainer bonded to the back of the supporting tooth to hold the pontic in place. They can be made of porcelain-fused-to-metal, all-ceramic materials (like lithium disilicate or zirconia) or plastic teeth and gums supported by a metal or ceramic framework.

A bridge is one way to fill gaps caused by missing teeth. It’s a good option if you:

  • Have small to moderate gaps (1-2 missing teeth).
  • Want a fixed tooth replacement (not removable).
  • Have healthy supporting teeth.
  • Are not a candidate for dental implants (e.g., due to medical conditions or insufficient bone).
  • Are under 18 and will get implants later.
  • Need a quick temporary replacement for a suddenly lost tooth (e.g., due to trauma).

What Does the Procedure Involve?

Stage 1: Consultation and Pre-Treatment

The dentist will talk to you, examine your teeth and take X-rays, measurements or pulp tests.

 

They will determine if a bridge (or another treatment) is right for you and discuss the different types of bridges.

 

Pre-treatment may include:

  • Core fillings to build up damaged supporting teeth.
  • Gum contouring to ensure the core filling and bridge fit properly.

Stage 3: Post-Operative Care

You’ll receive instructions on how to care for your bridge (including a special floss threader) and oral hygiene advice.

 

With proper care a bridge can last many years or a lifetime.

Stage 2: Bridge Procedure

Two appointments after pre-treatment. First appointment is for tooth preparation and impressions, second appointment is for bridge fitting.

 

Bridge Preparation

  • 30-90 minutes. Local anesthetic is used to numb the supporting teeth (unless a root canal has been done), so you’ll feel pressure/vibration, not pain. IV sedation or general anesthesia may be used depending on your needs and treatment plan.
  • If crowns are used, tooth structure is removed to make room for the bridge. If wings are used, the surfaces of the supporting teeth are cleaned and prepared.
  • An impression (or scan) is taken and sent to the lab to make the bridge. You’ll choose the shade with your dentist.
  • A temporary bridge or denture is placed to fill the gap while the permanent bridge is made. Don’t chew hard or sticky foods on the temporary bridge. Call your dentist if it breaks or comes off.

 

Bridge Fit

  • 20-45 minutes. The temporary bridge is removed and the permanent bridge is tried in. The dentist will check the fit, bite and shade.
  • Minor adjustments may be needed. The bridge is then cemented in. Local anesthetic is not usually required.
